Advanced wound management and wound medicine
Wound management is the hands-on work: debridement, dressings, compression, offloading, infection control. Wound medicine is the diagnostic work behind it — finding and fixing the reason the wound exists. Complex wounds need both, and we provide both at the bedside.
Mobile Wound Management
Full-service wound healing delivered at the bedside — private homes, assisted living, group homes, skilled nursing, and long-term care. No transportation, no waiting rooms, no missed appointments.
- In-home and in-facility visits on a set schedule
- Communication with your primary care provider and specialists
- Direct communication with family and facility staff
Advanced Wound Medicine
Wound medicine looks past the wound bed to the reason the wound exists. Perfusion, pressure, nutrition, glycemic control, infection, and medication effects are all part of the diagnosis.
- Comprehensive wound and lower-extremity assessment
- Coordination with mobile diagnostic companies for bedside vascular screening when perfusion is the suspected barrier
- Biopsy and workup for wounds that are not as they appear to be
Debridement & Dressing Selection
Healthy tissue cannot grow across non-living cells. We perform debridement as clinically indicated and match the dressing to the wound appearance, then update the plan as the wound changes.
- Autolytic, mechanical, and conservative sharp debridement
- Advanced dressings and antimicrobials
- Negative pressure wound therapy management
Compression & Offloading
Venous leg ulcers and diabetes-related foot wounds often persist when the underlying mechanical forces are not adequately addressed. We use compression when vascular status supports it and individualized offloading strategies to reduce pressure, shear, and repetitive tissue stress.
- Multi-layer compression wraps based on vascular assessment and clinical findings
- Individualized offloading plans using removable devices and pressure-relieving strategies when appropriate
- Pressure redistribution, positioning, and seating recommendations based on wound location and patient needs
Infection & Antimicrobial Stewardship
Wound infection is identified through clinical assessment, not culture results alone. We evaluate the patient and wound for signs of local, spreading, or systemic infection, obtain cultures when clinically indicated, and use targeted antimicrobial therapy when treatment is warranted.
- Assessment for local, spreading, and systemic signs of infection
- Culture when clinically indicated using an appropriate specimen and collection technique
- Evaluation for deeper infection or osteomyelitis, with timely escalation when needed
Facility & Home Health Partnership
We partner with facility and home health teams to strengthen prevention, for early identification, and consistent wound management between provider visits through practical education, communication, and clinical support.
- On-site education for nursing and care staff based on identified needs
- Support for skin and pressure injury prevention, wound observation, documentation, and escalation
- Case-based education and routine communication to support continuity between nursing staff and provider visits
Wound Management vs. Wound Medicine
Two halves of the same job
Wound management
The treatment happening at the wound itself, week to week. Done well, it protects healing tissue, removes what blocks it, and controls the local environment.
- Measurement, photography, and objective progress tracking
- Selective debridement of non-viable tissue
- Dressing selection matched to exudate, depth, and tissue type
- Periwound skin protection
- Negative pressure wound therapy oversight
- Supply coordination so the right products are actually on hand
Wound medicine
The diagnostic and systemic work. A wound is a symptom. If the cause is not identified and addressed, no dressing on the market will close it.
- Arterial and venous assessment, with vascular referral
- Neuropathy and biomechanical evaluation
- Nutrition and protein status review
- Glycemic control coordination
- Osteomyelitis evaluation and antimicrobial decisions
- Biopsy and workup for atypical, non-healing wounds
